415: Effectiveness of Erector Spinae or Paravertebral block in Micro-lumbar discectomy in Daycase surgery
Topic: Clinical Science
Subtopic: Pain category (ICD-11), Acute postsurgical pain, Treatment/management
Background and Aims: Postoperative pain management is crucial in day-case micro lumbar spine surgery to enhance early recovery and patient satisfaction. This study evaluates the effectiveness of lumbar paravertebral block (LPVB) and erector spinae plane block (ESPB) in providing postoperative analgesia.
Methods: A total of 50 patients undergoing micro lumbar spine surgery were randomly divided into two groups (n=25 each). Group A received an ESPB before incision, and Group B received an LPVB, using an identical local anesthetic solution consisting of 12.5 mL of 0.5% bupivacaine and 12.5 mL of 2% lignocaine with adrenaline (total 25 mL). Postoperative pain was assessed using the Visual Analog Scale (VAS) at 2, 6, 12, and 24 hours. Secondary outcomes included total opioid consumption, time to first analgesic request, hemodynamic stability, and side effects.
Results: Both blocks provided effective pain relief. The ESPB group showed significantly lower VAS scores at 6 and 12 hours postoperatively (p<0.05) and required less opioid consumption compared to the LPVB group. The time to first analgesic request was longer in the ESPB group. Hemodynamic stability was comparable in both groups, with no significant adverse effects reported.
Conclusions: Both lumbar paravertebral and erector spinae plane blocks are effective for postoperative analgesia in day-case micro lumbar spine surgery. However, ESPB demonstrated superior analgesic duration and reduced opioid consumption, making it a promising option for enhanced recovery in ambulatory spine procedures. Further studies with larger sample sizes are recommended to validate these findings.
