331: Therapeutic Potential of Platelet-Rich Plasma in Managing Knee Osteoarthritis: A Prospective Study
Topic: Clinical Science
Subtopic: Pain category (ICD-11), Chronic primary musculoskeletal pain, Pharmacological treatments, Non-pharmacological treatments, Physiotherapy
Background and Aims: Background: The regenerative effect of PRP is primarily linked to biomolecules in platelets alpha granules alond with a controlled inflammatory phase which is essential for healing. Leukocytes contributr to this plaseby releasing both pro and anti- inflammatory factors, supporting a process known as inflammatory regeneration . Macrophase plasticity , particularly the shift from M1 to M2 phenotype is plays a key role in this transition .This prospective study aimed to assess the effectiveness and safety of MIPSIs like PRP injections in Knee Osteoarthritis.
Aims: To evaluate ans assess the effectiveness of platelet rich plama injections on outcomes of Knee Osteoarthritis
Methods: A prospective study was conducted with total of 130 patients (72 females and 58 males) mean age range 40–70 years received a series of three PRP injections between January 2024 - August 2024 and completed all follow-up evaluations. Clinical outcomes were measured using the WOMAC and the VAS Score at baseline , one month , three and six months post-treatment. MRI and X-ray images were obtained at baseline and six months and included in the analysis.30 ml of blood was centrifuged and 5 ml of PRP was administered after every 3 weeks into suprapatellar recess under ultrasound guidance.
Results: Total 130 patient with knee arthritis were enrolled which included 72 males and 58 females with mean age of 61.7 (40-70 years).
BMI of these patients ranged between 22- 29.9 with mean of 24.05
The total WOMAC scores demonstrated a statistically significant decrease at multiple time points.
Specifically, there was a notable reduction from T0 to T3(28.5015.50 Vs 09.506.53 p < 0.0001)
There was significant improvement in VAS score too at multiple time points.
There was a notable reduction from from T0 to T3(07.500.50 Vs 02.500.29 p < 0.0001)
Conclusions: Conclusion: The results support the effectiveness of MIPSIs like intraarticular PRP injections under ultrasound guidance in managing KneeOA with pain reduction observed at one month following treatment and the most significant improvement at six months.
Our study demonstrated overall improvement in VAS and WOMAC score suggesting improvement in functionality and overall patient mobility.
Overall improvement in mobility could be attributed to multiple PRP injections.
The chondroprotective effects of PRP is linked to its ability to enhance the production of cartilage matrix and endogenous hyaluronic acid, offering pain relief and p slowing Osteoarthritis progression. It also exhibits anti-inflammatory and analgesic properties.
