453: Risk Factors for Pain Recurrence After Percutaneous Transforaminal Endoscopic Discectomy
Topic: Clinical Science
Subtopic: Pain category (ICD-11), Chronic primary musculoskeletal pain, Treatment/management
Background and Aims: Percutaneous transforaminal endoscopic discectomy (PTED) is a common minimally invasive treatment for lumbar disc herniation (LDH) in pain medicine, but pain recurrence affects prognosis. This study explores its risk factors and proposes targeted interventional strategies.
Methods: A retrospective analysis of 126 LDH patients who underwent PTED in the Pain Department of Liaocheng People’s Hospital was conducted. Patients were divided into recurrence and non-recurrence groups. Clinical data were collected, and univariate and multivariate Logistic regression analyses were performed.
Results: Univariate analysis showed significant differences in disease duration, BMI, annulus fibrosus damage, postoperative rehabilitation compliance, smoking history and diabetes between the two groups (P<0.05). Multivariate analysis identified these as independent risk factors (P<0.05).
Conclusions: Pain recurrence after PTED is affected by multiple factors. Comprehensive measures including preoperative evaluation, intraoperative protection, postoperative standardized analgesia, rehabilitation guidance, smoking cessation and basic disease management can reduce recurrence and improve prognosis.
