609: EXERGAMES IN THE MANAGEMENT OF MUSCULOSKELETAL DISORDERS IN OLDER ADULTS: A SYSTEMATIC REVIEW
Topic: Education, Societal Impact, and Others
Subtopic: Methodology (SLRs, Meta analysis)
Background and Aims: The evidence on the clinical effects of exergames in older adults has been studied and focused on neurological areas, mood disorders, mental health, cognitive aspects, and fall prevention. However, the effects of Exergames on the musculoskeletal area in this population is very unclear. In this research, we seek to explore the role of exergames on various physical, functional, and psychosocial variables.
Methods: A systematic review was reported according to the PRISMA statement was conducted. Randomized controlled trials (RCTs) were searched, by two independent reviewers, in Scopus, PubMed y Web of Science from inception to October 2025. Studies that applied non-immersive exergames and assessed physical functional, cognitive, pain, and psychosocial outcomes were included. Comparisons were other exercise modalities conventional therapy or non-intervention. Risk of bias of selected studies was assessed with the risk of bias RoB-2 tool.
Results: Eleven studies were included (total of participants = 564). Six studies had high RoB and 5 were rated as with some concern. Two studies involved knee OA, 5 were trials including a total knee replacement, one related to cervical pain, 2 studies included chronic musculoskeletal pain, and one on shoulder pain. All groups participated in exergames involving dynamic exercises. The most common durations of interventions lasted between four and six weeks.
Exergames showed positive effects. They demonstrated superiority over conventional therapy in specific areas, such as functional mobility, muscle strength, musculoskeletal pain, ROM, proprioception, aerobic endurance, and cervical kinematics.
Conclusions: Non-immersive exergames constitute an effective strategy for optimizing several relevant outcomes in rehabilitation. However, more RCTs with high methodological quality are required to deepen the knowledge about the multidimensional effects of exergames in the management of musculoskeletal disorders in older adults. However, the heterogeneity of the protocols, the low standardization of interventions, the small sample sizes, and the risk of bias of the studies limit the repercussions of these findings.
